Australian Primary Care Collaboratives (APCC)
The APCC is a quality improvement program assisting general practices and Aboriginal Medical Services in developing their capability to deliver rapid, sustainable and systematic improvement in the care they provide to patients and their communities. Clinics are able to monitor improvements and measure progress against other clinics locally and nationally.
 
Focus areas include:
·         Coronary Heart Disease
·         Diabetes, Patient Access to GPs
·         Chronic Obstructive Pulmonary Disease
·         Prevention and Chronic Condition Self Management
 
Due to funding arrangements the number of participating practices is limited. GPNNT will advertise for participation as new funding becomes available.
 
Participating Practices
Practices currently participating:
·         Nightcliff Medical Centre, Nightcliff
·         Bagot Health Clinic, NT Government, Darwin
·         The Bayside Clinic, Fannie Bay
·         Kalkarindji, Katherine West Health Board
·         Timber Creek, Katherine West Health Board
 
Over 20 clinics in the Northern Territory have already participated with huge achievements and successes, including: Alice Springs Family Medical Centre, Amoongana, Bagot Health Clinic, Mall Medical Centre, Nguiu Health Centre, Nightcliff Medical Centre, The Bayside Clinic, Casuarina Family Practice, Broom Remote Area Health Service, BMC, Dakas Street, Danila Dilba, Timber Creek, Kimberley Division of GP, Anyinginyi, Cavenagh, Congress, Central Clinic, Fred’s Pass Medical Centre.
