Improving Indigenous Access to Mainstream Primary Health Care Services

General Practice Network NT’s focus is to contribute to closing the gap by improving Aboriginal people’s access to mainstream primary health care services, through:  

  • tackling chronic disease risk factors
  • improving chronic disease management and follow-up
  • providing workforce training, expansion and support.

Support

GPNNT will support Aboriginal people with a chronic disease or those at risk of having a chronic disease to access primary health care services through employing Indigenous Outreach Workers and providing care coordination services .

GPNNT will support NT private general practices to:

  • deliver culturally appropriate services through providing cultural training appropriate to the needs of local Aboriginal community members
  • access Practice Incentive Program (PIP) Indigenous Health Incentives
  • provide culturally appropriate chronic disease health checks and management plans
  • encourage Aboriginal people to self-identify to appropriate mainstream services
  • understand Closing the Gap measures relevant to their clients and their practice
  • foster collaboration and support between mainstream primary care and the community controlled health sector.
